This Supplemental Privacy Notice is applicable to California, Colorado, Connecticut, Delaware, Iowa, Montana, New Hampshire, New Jersey, Oregon, Utah, and Virginia residents only, and is intended to supplement the Civis Privacy Policy.
The “Information We Collect About Individuals” section of our Privacy Policy outlines the categories and sources of personal information we collect and the “Our Purposes for Collecting the Information” section of our Privacy Policy describes how we use that information for business and commercial purposes.
The data privacy laws of the above listed states provide consumers with certain rights regarding their personal information, which may include the rights to access the personal information we have collected about them, (and related details in some cases, such as the sources of collection, the business/commercial purposes for use of such information, and the “third parties” that have received such information, where not already disclosed in the Civis Privacy Policy) correct such personal information if it is inaccurate, and delete personal information we collect about them, subject to exceptions.
These states also offer consumers similar forms of the rights to opt-out of “sales/shares” of personal information, “targeted advertising,” and certain processing of “sensitive” personal information. Some states require that we obtain your opt-in consent before processing such “sensitive” personal information.
Under these laws, consumers have the right not to receive discriminatory treatment for the exercise of their privacy rights.

A consumer or their authorized representative may submit a request to Civis to exercise these rights, including the opt-out rights (i.e., “sales/shares,” “targeted advertising,” and certain processing of “sensitive” personal information), by completing the Civis Data Subject Request Form. Alternatively, California consumers or their authorized representatives may submit requests at 877-788-5865. Where contacting us by phone, please provide the following information for the consumer: full name, mailing address, email address, telephone number, and type of rights request(s) being made. If this information is not provided, we may request additional information or confirmation, except where such verification is not permitted.
With respect to information collected through cookies and other tracking technologies on the Civis website that may result in “sales” or “shares” (or processing for “targeted advertising”), please use this link to opt-out of cookies. If you are a resident of California, Colorado,
Connecticut, Montana, Nebraska or New Hampshire, you may also visit our website using the Global Privacy Control (GPC) to opt-out of such cookies. These cookie opt-outs (whether by our toggles or by GPC) are on a per-website basis and apply at the browser level for the device you use to opt out.
In accordance with The California Delete Act, Civis discloses that it received and honored, within an average of 45 days, the privacy rights requests of 22 California residents in 2023.
If a consumer has elected to use an authorized representative, the authorized representative must submit proof of their agency relationship in the form of a valid power of attorney in compliance with their agent’s state law or reliable evidence of signed permission to act on the consumer’s behalf. For the security of personal information, we will verify the consumer’s identity directly with them prior to processing a request submitted by an authorized representative and we will only share the consumer’s personal information with the consumer directly.
We strive to respond to each data subject request within 45 days of its receipt (unless otherwise required under applicable law), but if we require more time, we will reach out via email. If we cannot comply with a request, we will explain why. We will provide a consumer with their personal information, if applicable, in an electronic format.
